**Enfuvirtide + optimized antiretroviral regimen** is a combination therapy for **treatment-experienced HIV-1-infected patients** with evidence of HIV-1 replication despite ongoing therapy. Enfuvirtide is a biomimetic peptide and the first fusion inhibitor to be FDA-approved for this indication. It binds to the first heptad-repeat (HR1) in the gp41 subunit of HIV-1, blocking conformational changes required for viral–cell membrane fusion, and thus inhibits viral entry into CD4+ T cells. This regimen is used in conjunction with an individualized (“optimized”) background of other antiretrovirals, typically in patients with limited options due to resistance. Enfuvirtide is administered subcutaneously.
