Eniluracil + fluorouracil + oxaliplatin is a combination chemotherapy regimen investigated for the treatment of advanced colorectal cancer, specifically within the FOX-E clinical trial sponsored by NYU Langone Health. The regimen centers on eniluracil (5-ethynyluracil), a potent and irreversible inhibitor of dihydropyrimidine dehydrogenase (DPD). DPD is the primary enzyme responsible for the rapid catabolism of 5-fluorouracil (5-FU); by inhibiting this enzyme, eniluracil allows for the oral administration of 5-FU and maintains therapeutic drug levels while potentially reducing the production of toxic metabolites like fluoro-beta-alanine. This combination also includes oxaliplatin, a platinum-based antineoplastic agent that forms DNA adducts to inhibit DNA replication and transcription. The FOX-E study was a Phase I/II dose-escalation trial designed to evaluate the safety and efficacy of this three-drug combination in patients who had previously received treatment for advanced colorectal cancer.
