Overview

This combination regimen consists of the low molecular weight heparin (LMWH) enoxaparin administered concomitantly with standard chemotherapy. It was specifically evaluated by the Thrombosis Research Institute in the Phase IIIb GASTRANOX trial for the treatment of inoperable gastric and gastroesophageal cancer. Enoxaparin is a small molecule anticoagulant that functions by binding to and accelerating the activity of antithrombin III, which in turn inhibits activated factor X (Xa) and, to a lesser degree, thrombin (factor IIa). In the context of oncology, this regimen is based on the hypothesis that LMWHs may provide survival benefits beyond their anticoagulant effects, potentially through anti-tumor and anti-metastatic biological activities. In the GASTRANOX study, enoxaparin was administered at a dose of 1 mg/kg (50% of the full treatment dose) for six months alongside six cycles of investigator-selected standard chemotherapy.
