Enoxaparin + unfractionated heparin is a combination of two anticoagulant drugs used to prevent and treat thromboembolic disorders. Enoxaparin is a low molecular weight heparin (LMWH) that acts primarily by binding to antithrombin III, enhancing its inhibition of factor Xa and, to a lesser extent, factor IIa (thrombin), thereby preventing the formation of fibrin clots[3][5]. Unfractionated heparin (UFH) is a heterogeneous mixture of glycosaminoglycan polymers that also binds to antithrombin III but inhibits both factor Xa and thrombin more equally[6][8]. Both agents are used for prophylaxis and treatment of deep vein thrombosis, pulmonary embolism, acute coronary syndromes such as unstable angina or myocardial infarction, and during procedures requiring anticoagulation. Enoxaparin has more predictable pharmacokinetics than UFH and does not require routine monitoring in most cases; UFH requires close monitoring due to variable response. The combination may be considered in certain clinical scenarios where transitioning between agents or bridging therapy is required.
