Overview

Enoximone + esmolol is a pharmacological combination therapy investigated for perioperative cardiac protection in high-risk patients. Enoximone is a selective phosphodiesterase 3 (PDE3) inhibitor that exerts positive inotropic and vasodilatory effects by increasing intracellular cyclic adenosine monophosphate (cAMP) levels. Esmolol is an ultra-short-acting, cardioselective beta-1 adrenergic receptor antagonist used for rapid heart rate control. The combination was studied by researchers at Klinikum Ludwigshafen to mitigate the risk of myocardial ischemia and heart failure in patients with coronary artery disease undergoing major vascular surgery. The rationale for the combination is to utilize esmolol for heart rate control and reduction of myocardial oxygen demand, while enoximone provides inotropic support and vasodilation to maintain cardiac output and counteract potential beta-blocker-induced myocardial depression. The therapy is typically administered via continuous intravenous infusion during the perioperative period.
