Envafolimab + gemcitabine + cisplatin is a multi-agent chemotherapy and immunotherapy regimen under investigation primarily for advanced biliary tract cancer (BTC) and other solid tumors. Envafolimab is a novel fusion protein consisting of a humanized single-domain PD-L1 antibody fused to an IgG1 Fc fragment; it acts as an immune checkpoint inhibitor by blocking the interaction between PD-L1 on tumor cells and PD-1 on T cells, thereby enhancing anti-tumor immune responses[8][6]. Gemcitabine is a nucleoside analog antimetabolite that inhibits DNA synthesis in rapidly dividing cells[7]. Cisplatin is a platinum-based chemotherapeutic agent that forms DNA crosslinks leading to apoptosis in cancer cells[2]. The combination leverages both cytotoxic effects from chemotherapy (gemcitabine/cisplatin) and immune modulation from envafolimab. This regimen has shown promising efficacy as first-line therapy in advanced BTC according to ongoing phase II studies[1][4].
