enzalutamide + dutasteride + finasteride-University of Rochester-prostate cancer is a combination therapy regimen being evaluated by the University of Rochester in a Phase II clinical trial (NCT02213107). The study investigates the efficacy of enzalutamide, a potent androgen receptor antagonist, in combination with a 5α-reductase inhibitor—either dutasteride or finasteride—as a first-line treatment for vulnerable elderly patients (aged 65 and older) with systemic hormone-naive prostate cancer. Enzalutamide works by inhibiting androgen receptor ligand binding, nuclear translocation, and DNA binding. Dutasteride and finasteride function by blocking the conversion of testosterone to dihydrotestosterone (DHT), the primary driver of prostate cancer growth. This dual-action approach aims to provide a more tolerable alternative to standard androgen deprivation therapy (ADT) by achieving a medical prostatectomy while potentially mitigating the systemic side effects associated with conventional hormone therapy in older populations.
