enzalutamide + PSA-TRICOM

enzalutamide + PSA-TRICOM is a combination therapy being investigated for the treatment of prostate cancer, specifically non-metastatic castration-sensitive prostate cancer. Enzalutamide is a potent androgen receptor inhibitor that blocks multiple steps in the androgen receptor signaling pathway, including androgen binding, nuclear translocation, and DNA binding, thereby inhibiting the growth of prostate cancer cells. PSA-TRICOM (also known as PROSTVAC) is a therapeutic cancer vaccine platform utilizing two different live-attenuated viral vectors—a vaccinia virus (PROSTVAC-V) for priming and a fowlpox virus (PROSTVAC-F) for boosting. Both vectors are engineered to express human prostate-specific antigen (PSA) along with a triad of costimulatory molecules (B7.1, ICAM-1, and LFA-3), collectively termed TRICOM, which are designed to enhance the activation of T cells against PSA-expressing tumor cells. The combination aims to leverage the hormonal blockade of enzalutamide alongside the immune-stimulatory effects of the vaccine to improve clinical outcomes.
