enzalutamide + toripalimab + testosterone is an experimental combination therapy regimen being investigated for the treatment of metastatic castration-resistant prostate cancer (mCRPC). This regimen incorporates Bipolar Androgen Therapy (BAT), which involves the administration of high-dose testosterone to rapidly cycle serum levels between supraphysiologic and castrate concentrations. This cycling is intended to disrupt androgen receptor (AR) signaling and induce DNA damage in prostate cancer cells. The regimen further includes enzalutamide, a second-generation AR antagonist that inhibits androgen binding, nuclear translocation, and DNA binding, and toripalimab, a humanized monoclonal antibody targeting the programmed cell death protein 1 (PD-1) receptor. The combination aims to leverage the potential of BAT to sensitize tumors to AR inhibition and enhance the anti-tumor immune response facilitated by checkpoint blockade. This specific triplet is primarily being developed and studied by researchers at Beijing Hospital.
