Overview

Epacadostat + ipilimumab is a combination of two immunotherapy agents studied for advanced melanoma. **Epacadostat** is a potent, selective oral inhibitor of indoleamine 2,3-dioxygenase 1 (IDO1)—an intracellular enzyme that degrades tryptophan and promotes tumor-induced immunosuppression. **Ipilimumab** is a monoclonal antibody targeting cytotoxic T-lymphocyte-associated protein 4 (CTLA-4), enhancing T-cell activation and anti-tumor immune responses. The combination was evaluated in a phase 1 trial for unresectable or metastatic melanoma; while lower doses of epacadostat combined with ipilimumab were generally tolerable and active, higher epacadostat doses produced unacceptable hepatotoxicity. The trial did not progress to phase 2 due to changes in clinical strategy, not safety concerns.
