This is a combination investigational regimen consisting of **epacadostat**, an oral small molecule inhibitor of indoleamine 2,3-dioxygenase 1 (**IDO1**); **nivolumab**, a monoclonal antibody targeting programmed cell death protein 1 (**PD-1**); and **ipilimumab**, a monoclonal antibody targeting cytotoxic T-lymphocyte-associated protein 4 (**CTLA-4**). - **Epacadostat** works by blocking IDO1, an enzyme that depletes tryptophan in the tumor microenvironment, leading to reduced immunosuppression and enhanced T-cell responses. - **Nivolumab** inhibits PD-1, removing a checkpoint that suppresses the immune response to tumor cells. - **Ipilimumab** blocks CTLA-4, another checkpoint receptor on T cells, enhancing immune activation against tumors. This triple combination is under early clinical investigation for the treatment of advanced or metastatic solid tumors, including melanoma and non-small cell lung cancer. The aim is to provide synergistic immunomodulatory effects by targeting multiple checkpoints and immunosuppressive pathways[4][5].
