epalrestat (Kyorin Pharmaceutical)

Epalrestat is a carboxylic acid derivative that inhibits aldose reductase, a rate-limiting enzyme of the polyol pathway. Under hyperglycemic conditions, epalrestat reduces intracellular sorbitol accumulation, which has been implicated in the pathogenesis of late-onset complications of diabetes mellitus, particularly diabetic neuropathy. It is primarily used to improve subjective symptoms (numbness, pain, hyperesthesia), abnormal vibration sense, and abnormal heart rate variability associated with diabetic peripheral neuropathy.
