A combination therapy consisting of epalrestat, an aldose reductase inhibitor, and alpha lipoic acid (ALA), a potent antioxidant. This combination is primarily used for treating diabetic peripheral neuropathy (DPN), a common long-term complication of diabetes mellitus. This combination therapy has shown superior efficacy compared to either medication used alone. ## Mechanism of Action Epalrestat is an aldose reductase inhibitor that works by reducing the expression of antioxidant enzymes and aldose reductase, alleviating oxidative stress, and suppressing the polyol pathway[3]. This helps prevent endothelial cell function damage[8]. Alpha lipoic acid (ALA) is a potent antioxidant that directly eliminates free radicals, inhibits peroxidation, enhances blood flow, increases nerve Na+–K+ ATPase activity, and protects endothelial function[3]. It can also mobilize bound mercury into the bloodstream as it is a mercaptan (sulfur compound that readily binds to mercury)[6]. ## Clinical Efficacy Multiple studies have demonstrated that the combination of epalrestat and ALA provides better outcomes than monotherapy with either agent. A meta-analysis of randomized controlled trials showed that: - The combined treatment exhibited a better total effectiveness rate than epalrestat alone (RR: 1.40, 95% CI: 1.16–1.69, P=0.0005 for 14 days of treatment; RR: 1.48, 95% CI: 1.27–1.72, P<0.00001 for 28 days of treatment)[1][2]. - The combination therapy significantly improved nerve conduction velocities, including median motor nerve conduction velocity (MNCV), sensory nerve conduction velocity (SNCV), peroneal MNCV, and peroneal SNCV after various treatment durations[2][5]. - The combination also reduced Toronto Clinical Scoring System (TCSS) scores (WMD: -1.60, 95% CI: -2.91, -0.29, P=0.02) and Total Symptom Score (TSS) (WMD: -0.93, 95% CI: -1.27, -0.60, P<0.00001) after 21 days of treatment[2]. ## Dosage Regimens Common dosage regimens studied include: - ALA 600 mg/day once daily combined with epalrestat 50 mg three times daily for 14, 21, or 28 days[1][2] - ALA 300 mg/day once daily combined with epalrestat 50 mg three times daily for 28 days[1][2] ## Other Applications Beyond diabetic peripheral neuropathy, research has also explored the protective effects of alpha-lipoic acid and epalrestat on oxaliplatin-induced peripheral neuropathy, suggesting potential applications in preventing chemotherapy-induced neuropathy[4]. The combination has also been studied alongside other therapies such as electroacupuncture for enhanced efficacy in treating DPN and its effects on nerve conduction velocity and oxidative stress[8].
