Epalrestat + mecobalamin is a fixed-dose combination of two active pharmaceutical ingredients used primarily for the management of diabetic peripheral neuropathy. Epalrestat is an aldose reductase inhibitor that prevents the conversion of glucose to sorbitol, thereby reducing sorbitol accumulation in nerve cells—a key factor in diabetic neuropathy pathogenesis. By inhibiting this pathway, epalrestat helps prevent neuronal damage and improves nerve conduction velocity. Mecobalamin (methylcobalamin) is a biologically active form of vitamin B12 that supports nerve regeneration by promoting myelin sheath formation and repair, thus aiding in the recovery and protection of damaged nerves. The combination has demonstrated synergistic effects—reducing inflammatory markers, improving electromyographic indices, decreasing oxidative stress responses, and providing superior symptomatic relief compared to monotherapy with either agent alone[1][3][5]. This drug is administered orally as tablets or capsules and is indicated for adult patients with diabetic neuropathy.
