Epinephrine + indomethacin is a combination pharmacological regimen being investigated by Duzce University for the prevention of post-endoscopic retrograde cholangiopancreatography pancreatitis (PEP). The regimen involves the preprocedural administration of rectal indomethacin, a non-steroidal anti-inflammatory drug (NSAID) that inhibits cyclooxygenase (COX) enzymes to reduce pancreatic inflammation, followed by the post-procedural injection of epinephrine into the duodenal papilla. Epinephrine acts as an adrenergic receptor agonist, inducing local vasoconstriction to minimize papillary edema and mechanical trauma caused during the ERCP procedure. This dual-action approach aims to address both the inflammatory and mechanical pathways of PEP pathogenesis.
