Overview

epinephrine + methylprednisolone is an investigational combination therapy being evaluated for the treatment of out-of-hospital cardiac arrest (OHCA). This specific regimen was designated as Experimental Group 2 in the VSE Study conducted by Korea University Guro Hospital. Epinephrine is a non-selective adrenergic agonist that increases systemic vascular resistance and improves coronary and cerebral perfusion pressure during cardiopulmonary resuscitation. Methylprednisolone is a synthetic glucocorticoid with potent anti-inflammatory properties; it is included in the protocol to mitigate the systemic inflammatory response associated with post-cardiac arrest syndrome and to potentially address relative adrenal insufficiency during the stress of cardiac arrest. The combination aims to improve the rates of return of spontaneous circulation (ROSC) and long-term neurological outcomes compared to standard epinephrine monotherapy.
