Overview

This is a combination of five cardiovascular agents—epinephrine, norepinephrine, dobutamine, dopamine, and levosimendan—used for advanced hemodynamic support in critical care settings. Each component acts as a positive inotrope or vasopressor with distinct mechanisms: - Epinephrine and norepinephrine are endogenous catecholamines that stimulate adrenergic receptors to increase cardiac output and vascular tone. - Dobutamine is a synthetic catecholamine primarily stimulating beta-1 adrenergic receptors to enhance myocardial contractility. - Dopamine acts on dopaminergic and adrenergic receptors depending on dose, providing both inotropic support and vasoconstriction. - Levosimendan is an inodilator that increases cardiac contractility by sensitizing troponin C to calcium (without increasing intracellular calcium) and causes vasodilation by opening ATP-sensitive potassium channels[1][2][3]. This combination may be considered for patients with severe acute or decompensated heart failure or shock states unresponsive to standard therapy.
