Overview

This is a multi-agent chemotherapy regimen consisting of the anthracycline antibiotic epirubicin, the alkylating agent cyclophosphamide, and the recombinant human granulocyte colony-stimulating factor filgrastim, used together primarily in breast cancer to enable dose-intense or dose-dense epirubicin–cyclophosphamide treatment while limiting neutropenia and febrile neutropenia. Epirubicin intercalates into DNA and inhibits topoisomerase II, generating DNA breaks and free radicals that lead to tumor cell death, while cyclophosphamide is a prodrug activated in the liver to phosphoramide mustard, which forms DNA crosslinks and interferes with replication.[1][3][4] Filgrastim (G-CSF) stimulates proliferation and differentiation of neutrophil precursors and enhances neutrophil function, permitting higher doses or shortened intervals of epirubicin–cyclophosphamide by reducing the depth and duration of neutropenia.[1][2][3][4] The combination has been investigated and used in adjuvant, neoadjuvant, and advanced breast cancer to escalate dose intensity or decrease intertreatment intervals, supported by filgrastim to maintain deliverability and manage hematologic toxicity.[1][2][3][4]
