Equicabtagene autoleucel + selinexor is a combination therapy being evaluated for the treatment of relapsed or refractory extramedullary multiple myeloma. **Equicabtagene autoleucel (CT103A)** is a chimeric antigen receptor (CAR) T-cell therapy that targets the B-cell maturation antigen (BCMA) using a fully human single-chain variable fragment (scFv), a CD8a hinge and transmembrane domain, and 4-1BB co-stimulatory and CD3ζ activation domains. **Selinexor** is a first-in-class, oral selective inhibitor of nuclear export (SINE) that specifically binds to and inhibits Exportin 1 (XPO1), leading to the nuclear retention and activation of tumor suppressor proteins. This combination is being studied in a Phase I clinical trial led by Chunrui Li at Tongji Hospital to determine if the synergistic effects of XPO1 inhibition and BCMA-targeted CAR-T cell activity can improve outcomes in patients with aggressive extramedullary disease, who typically exhibit poor responses to standard CAR-T monotherapy.
