Eribulin mesylate + trastuzumab biosimilar SB3 is a combination therapy being investigated for the treatment of HER2-positive recurrent or metastatic breast cancer. The regimen consists of eribulin mesylate (Halaven), a synthetic analogue of halichondrin B that acts as a non-taxane microtubule dynamics inhibitor, and SB3 (Samfenet/Ontruzant), a biosimilar of the monoclonal antibody trastuzumab. Eribulin inhibits the growth phase of microtubules without affecting the shortening phase, sequestering tubulin into non-productive aggregates and inducing G2/M cell cycle arrest. SB3 targets the extracellular domain of the Human Epidermal Growth Factor Receptor 2 (HER2/ERBB2), blocking downstream proliferative signaling and inducing antibody-dependent cellular cytotoxicity (ADCC). This combination is specifically being evaluated in the Phase II ESPERO trial as a salvage therapy for patients who have previously failed standard HER2-directed treatments, including trastuzumab and ado-trastuzumab emtansine (T-DM1).
