Overview

erythromycin + asparaginase refers to the concomitant use of the macrolide antibiotic erythromycin with the antineoplastic enzyme asparaginase, most commonly in the context of multiagent chemotherapy regimens where asparaginase is used to deplete circulating asparagine and inhibit protein synthesis in leukemic lymphoblasts. Erythromycin acts by binding the 50S ribosomal subunit of susceptible bacteria to inhibit protein synthesis, and in oncology settings is typically given to prevent or treat intercurrent bacterial infections during intensive chemotherapy. Asparaginase, including formulations such as native Escherichia coli asparaginase, Erwinia-derived asparaginase, pegylated asparaginase, or erythrocyte-encapsulated asparaginase (eryaspase), catalyzes the hydrolysis of asparagine to aspartic acid and ammonia, leading to systemic asparagine depletion that selectively impairs malignant lymphoblasts and certain solid tumors like pancreatic adenocarcinoma. The use of erythromycin with asparaginase does not represent a distinct fixed-dose combination product but rather a pharmacologic coadministration that may raise concerns about overlapping hepatotoxicity and alterations in drug metabolism during complex chemotherapy regimens.
