Erythromycin + atropine is a combination of two pharmacologically active agents. Erythromycin is a macrolide antibiotic that inhibits bacterial protein synthesis by binding to the 50S ribosomal subunit, thereby preventing the growth of susceptible bacteria. It is primarily used to treat and prevent various bacterial infections[1][4]. Atropine is an anticholinergic (muscarinic acetylcholine receptor antagonist) that blocks the action of acetylcholine on muscarinic receptors, leading to decreased secretions, increased heart rate, relaxation of smooth muscle in the airways and gastrointestinal tract, and mydriasis[2][7]. While both drugs are well-established individually for their respective indications—erythromycin as an antibiotic and prokinetic agent; atropine as an antimuscarinic for bradycardia, organophosphate poisoning, pre-anesthetic medication, and ophthalmic uses—their direct fixed-dose combination does not appear to be marketed or widely used clinically. However, there is evidence from research studies that atropine can antagonize some prokinetic effects of erythromycin on gastrointestinal motility by blocking cholinergic pathways[5][7].
