Overview

Erythromycin + gentamicin is a combination of two antibiotics used together for their complementary antibacterial effects. Erythromycin is a macrolide antibiotic that inhibits bacterial protein synthesis by binding to the 50S ribosomal subunit, primarily effective against gram-positive bacteria and some gram-negative organisms. Gentamicin is an aminoglycoside antibiotic that binds to the 30S ribosomal subunit, causing misreading of mRNA and inhibiting protein synthesis, with strong activity against aerobic gram-negative bacteria. The combination may be used in clinical scenarios where broad-spectrum coverage is needed or when synergy between the drugs can improve bacterial eradication, such as in certain intra-abdominal infections or experimental models like acute appendicitis[3]. Erythromycin can suppress aerobic flora other than Enterobacteriaceae and reduce counts of anaerobic Gram-negative bacteria; gentamicin adds further suppression, especially against resistant organisms[3]. Both drugs are administered parenterally (gentamicin) or orally/parenterally (erythromycin), depending on indication.
