Overview

The combination of erythromycin and metoclopramide is a dual-prokinetic therapy used primarily in the intensive care unit (ICU) to manage gastric feeding intolerance in critically ill patients. Erythromycin, a macrolide antibiotic, acts as a motilin receptor agonist, mimicking the effects of the endogenous hormone motilin to induce phase III migrating motor complex-like contractions in the stomach. Metoclopramide is a benzamide that functions as a dopamine D2 receptor antagonist and a 5-HT4 receptor agonist, which enhances upper gastrointestinal motility and accelerates gastric emptying. When used together, these agents provide a synergistic effect on gastric motility, often succeeding in patients who fail monotherapy. This specific combination has been extensively studied by researchers at the Guangdong Provincial People's Hospital and the Guangdong Academy of Medical Sciences to optimize enteral nutrition delivery in patients with critical illness.
