Overview

Erythromycin is a well-established macrolide antibiotic being evaluated in the Preterm Rupture of Membranes Optimising Antibiotics Trial (PROMOAT), a clinical study led by the University of Melbourne. This trial is a domain within the larger PLATIPUS adaptive platform trial (NCT06461429). In this specific intervention arm, erythromycin is administered at a dose of 250mg orally, paired with a placebo to maintain the double-blind nature of the study against other antibiotic regimens. The primary therapeutic goal is to prevent maternal and neonatal infections in cases of preterm prelabour rupture of membranes (PPROM) occurring before 37 weeks' gestation. Erythromycin functions by binding to the 50S ribosomal subunit of bacteria, specifically the 23S rRNA, which inhibits RNA-dependent protein synthesis and halts bacterial growth.
