This combination regimen consists of the escalated BEACOPP (bleomycin, etoposide, doxorubicin, cyclophosphamide, vincristine, procarbazine, and prednisone) chemotherapy protocol administered alongside goserelin, a gonadotropin-releasing hormone (GnRH) agonist. Developed and studied by the University of Cologne and the German Hodgkin Study Group (GHSG) in the PROFE trial, the regimen was designed to treat advanced-stage Hodgkin lymphoma while investigating the potential of goserelin to protect ovarian function and reduce treatment-induced infertility in fertile women. Goserelin works by temporarily suppressing the pituitary-gonadal axis, theoretically placing the ovaries in a quiescent state to shield them from the cytotoxic effects of chemotherapy. However, results from the PROFE trial indicated that goserelin did not significantly reduce ovarian toxicity compared to oral contraceptives in this specific setting.
