This is a combination of three pharmaceutical drugs—escitalopram, fluoxetine, and methylphenidate. Escitalopram and fluoxetine are both selective serotonin reuptake inhibitors (SSRIs) used primarily to treat depression and anxiety disorders by increasing serotonin levels in the brain through inhibition of its reuptake at the synaptic cleft. Methylphenidate is a central nervous system stimulant commonly prescribed for attention-deficit/hyperactivity disorder (ADHD) and narcolepsy; it works mainly by inhibiting dopamine and norepinephrine reuptake, thereby increasing their concentrations in the synaptic cleft. While combinations of an SSRI with methylphenidate are sometimes used clinically for patients with comorbid ADHD and depression, there is no established clinical precedent or approval for combining two SSRIs (escitalopram plus fluoxetine) together with methylphenidate due to overlapping mechanisms, increased risk of adverse effects such as serotonin syndrome, QT prolongation, or other pharmacodynamic interactions[1][2][3][4][6][8]. The safety profile of combining one SSRI with methylphenidate has been studied; however, triple therapy involving two SSRIs plus a stimulant lacks evidence for efficacy or safety.
