Overview

Escitalopram + mirtazapine is a combination of two antidepressant medications used together, most commonly in the management of major depressive disorder and sometimes for treatment-resistant depression. Escitalopram is a selective serotonin reuptake inhibitor (SSRI) that increases serotonin levels in the brain by inhibiting its reuptake into presynaptic cells. Mirtazapine is a noradrenergic and specific serotonergic antidepressant (NaSSA) that enhances central noradrenergic and serotonergic activity primarily through antagonism of central alpha-2 adrenergic receptors and certain serotonin receptors. The rationale for combining these drugs is to potentially achieve greater efficacy or faster onset of action than monotherapy, especially in patients who do not respond adequately to single-drug treatment. However, evidence on the benefit of this combination is mixed; some studies suggest improved remission rates while others show no significant advantage over monotherapy[5][6][7]. The combination may increase the risk of additive side effects such as sedation, impairment in judgment or psychomotor skills, and rarely, serotonin toxicity[1][4].
