escitalopram + mirtazapine + venlafaxine

This is a non-standard, off-label combination of three antidepressant medications—escitalopram (a selective serotonin reuptake inhibitor, SSRI), mirtazapine (a noradrenergic and specific serotonergic antidepressant, NaSSA), and venlafaxine (a serotonin-norepinephrine reuptake inhibitor, SNRI). Each drug targets different aspects of monoamine neurotransmission in the brain. Escitalopram primarily inhibits the reuptake of serotonin; venlafaxine inhibits both serotonin and norepinephrine reuptake; mirtazapine antagonizes central presynaptic alpha-2 adrenergic receptors as well as certain postsynaptic serotonergic receptors. The rationale for combining these agents is to achieve broader modulation of neurotransmitter systems in treatment-resistant depression. However, there is limited evidence supporting the efficacy or safety of this triple combination over monotherapy or dual therapy regimens. Case reports have described increased risk for adverse effects such as bleeding when all three are used together[7][2]. This regimen should only be considered under specialist supervision due to potential risks including pharmacodynamic interactions and side effect burden.
