Escitalopram is a selective serotonin reuptake inhibitor that increases serotonergic neurotransmission by inhibiting serotonin reuptake, while venlafaxine is a serotonin-norepinephrine reuptake inhibitor that inhibits serotonin and norepinephrine reuptake. The combination may be considered only under clinician-directed antidepressant management, such as during a carefully supervised treatment transition or selected treatment-resistant cases; overlapping serotonergic activity can increase the risk of serotonin syndrome and other additive adverse effects.
