esketamine + magnesium sulfate is a combination therapy being investigated for the prevention and treatment of postoperative fatigue syndrome (POFS), particularly in patients undergoing laparoscopic cholecystectomy. Esketamine, the S-enantiomer of ketamine, acts as a non-competitive N-methyl-D-aspartate (NMDA) receptor antagonist, which helps mitigate central sensitization and neuroinflammation by blocking microglial IL-6 release and enhancing neuroplasticity via BDNF/TrkB upregulation. Magnesium sulfate serves as a physiological NMDA receptor antagonist and a blocker of voltage-gated calcium channels, providing complementary effects by reducing calcium overload and supporting cellular energy metabolism as an ATPase cofactor. This combination aims to address both the central neurological and peripheral metabolic components of postoperative fatigue to accelerate rehabilitation.
