Overview

Esketamine + remifentanil is a drug combination being investigated for procedural sedation and analgesia, specifically for patients undergoing colonoscopy. This combination leverages the synergistic effects of esketamine, the (S)-enantiomer of ketamine which acts as a non-competitive N-methyl-D-aspartate (NMDA) receptor antagonist, and remifentanil, an ultra-short-acting mu-opioid receptor agonist. Esketamine provides potent analgesia and dissociative anesthesia with a more favorable side-effect profile regarding respiratory depression compared to racemic ketamine. Remifentanil offers rapid onset and offset of analgesia, allowing for precise control during the procedure and quick recovery. The combination is primarily studied by the Anesthesiology Department of Northern Jiangsu People's Hospital to optimize patient comfort and safety during gastrointestinal endoscopic procedures.
