Overview

Esmolol + milrinone is a combination therapy consisting of two intravenous cardiovascular drugs used primarily in critical care settings, especially for patients with severe sepsis or septic shock. Esmolol is an ultra-short-acting selective beta-1 adrenergic receptor antagonist (beta-blocker) that reduces heart rate and myocardial oxygen demand. Milrinone is a phosphodiesterase III inhibitor that acts as an inotrope and vasodilator, increasing cardiac contractility and reducing afterload. The combination aims to optimize cardiac function by controlling tachycardia (via esmolol) while supporting contractility and perfusion (via milrinone). Clinical studies have shown this combination can improve cardiac function, control heart rate more effectively than monotherapy, and increase 28-day survival rates in patients with severe sepsis[1][5][6]. This regimen is typically administered intravenously under close monitoring in intensive care units.
