Overview

This is a four-drug combination therapy consisting of esomeprazole (a proton pump inhibitor), bismuth subcitrate (a gastrointestinal protectant with antimicrobial activity), amoxicillin (a beta-lactam antibiotic), and metronidazole (a nitroimidazole antibiotic). The regimen is used primarily for the eradication of Helicobacter pylori infection, especially in cases where resistance or intolerance to other regimens exists. Esomeprazole reduces gastric acid secretion, enhancing the effectiveness of antibiotics against H. pylori. Bismuth subcitrate provides direct antimicrobial action and mucosal protection. Amoxicillin disrupts bacterial cell wall synthesis, while metronidazole causes DNA strand breakage in anaerobic bacteria like H. pylori[3][4][6]. This quadruple therapy is an alternative to classic bismuth-based regimens that use tetracycline instead of amoxicillin.
