This feminizing hormone therapy regimen consists of a combination of estradiol valerate and spironolactone, specifically utilized in the Féminas implementation science study conducted by Asociación Civil Impacta Salud y Educación (IMPACTA) in Lima, Peru. The intervention was designed to provide gender-affirming medical care integrated with HIV prevention and treatment services for transgender women. Estradiol valerate, a synthetic estrogen, acts as an agonist at the estrogen receptor to induce feminization. Spironolactone, while primarily a mineralocorticoid receptor antagonist, is used in this context for its secondary role as an androgen receptor antagonist to suppress masculine secondary sexual characteristics. The study (NCT03757117) aimed to determine if providing these hormones alongside peer navigation could improve the HIV treatment cascade, particularly viral suppression, in this high-burden population.
