The combination of estrogen-based Hormone Replacement Therapy (HRT) and autologous Platelet-Rich Plasma (PRP) is an experimental protocol developed by the Azienda Sanitaria Locale di Asti for treating recurrent implantation failure (RIF) and refractory thin endometrium. In this therapeutic approach, standard HRT is used to prepare the endometrial lining for embryo transfer. For patients showing an insufficient response (endometrial thickness <7 mm), autologous PRP—prepared from the patient's own blood via double centrifugation—is administered through intrauterine infusion. PRP is rich in essential growth factors, including vascular endothelial growth factor (VEGF), platelet-derived growth factor (PDGF), and transforming growth factor beta (TGF-beta), which promote angiogenesis, cellular proliferation, and tissue regeneration. This combined strategy aims to enhance endometrial receptivity and improve clinical pregnancy outcomes in women who have previously failed conventional assisted reproductive technology (ART) treatments.
