This is a combination therapy consisting of etanercept, GAD-alum (glutamic acid decarboxylase formulated with alum, also known as Diamyd), and vitamin D (calciferol). Etanercept is a recombinant soluble tumor necrosis factor-alpha (TNF-α) receptor fusion protein that binds to TNF-α and blocks its biological activity, thereby reducing inflammation. GAD-alum is an antigen-specific immunotherapy designed to induce immune tolerance by presenting the autoantigen glutamic acid decarboxylase 65 (GAD65) in an alum adjuvant formulation. Vitamin D has immunomodulatory effects, including promoting Th2 deviation, improving dendritic cell function, increasing regulatory T cells, and potentially protecting pancreatic beta cells. This combination was studied in children and adolescents with newly diagnosed type 1 diabetes to assess feasibility, safety, tolerability, and preservation of beta cell function. The regimen was found feasible and safe but did not show beneficial effects on the autoimmune process or beta cell preservation[1][2].
