Etanercept + methylprednisolone is a combination therapy investigated for the treatment of acute idiopathic pneumonia syndrome (IPS) following allogeneic hematopoietic cell transplantation (HCT). Etanercept is a dimeric fusion protein consisting of the extracellular ligand-binding portion of the human 75 kilodalton (p75) tumor necrosis factor receptor (TNFR) linked to the Fc portion of human IgG1. It functions as a TNF-alpha inhibitor by competitively binding to TNF-alpha and preventing its interaction with cell surface receptors. Methylprednisolone is a synthetic glucocorticoid with potent anti-inflammatory and immunosuppressive properties. The combination was evaluated in the Phase 3 BMT CTN 0403 trial, sponsored by the National Heart, Lung, and Blood Institute (NHLBI), but the study was terminated early due to poor patient accrual.
