**etentamig + lenalidomide** is a combination regimen under investigation for the treatment of multiple myeloma, especially in patients with relapsed or refractory disease. Etentamig (ABBV-383) is a bispecific monoclonal antibody designed as a T-cell engager targeting B-cell maturation antigen (BCMA). It recruits and activates T-cells to induce cytotoxicity specifically in BCMA-expressing malignant plasma cells. Lenalidomide is an oral immunomodulatory small molecule that inhibits the proliferation of malignant cells, enhances immune cell function, and modulates cytokine production. It acts via cereblon-mediated E3 ubiquitin ligase complex, leading to the degradation of transcription factors key to plasma cell survival, and also boosts NK cell activity. The combination is designed to synergistically enhance anti-myeloma activity by simultaneous immune-mediated tumor cell destruction and direct tumor cell killing. This regimen is presently in early-phase clinical trials.
