ethambutol + isoniazid + pyrazinamide + rifampicin (Natrapharm)

Fixcom 4 (brand name: Fixcom® 4) is an approved, prescription-only, fixed-dose combination antituberculosis small molecule drug developed and distributed by Natrapharm in the Philippines. It contains four active ingredients: Rifampicin (150 mg), Isoniazid (75 mg), Pyrazinamide (400 mg), and Ethambutol (275 mg), formulated as a film-coated tablet. It is indicated for the initial or intensive phase treatment of all forms of pulmonary and extrapulmonary tuberculosis. The combination targets Mycobacterium tuberculosis through multiple synergistic mechanisms: Rifampicin inhibits bacterial RNA polymerase, Isoniazid inhibits mycolic acid synthesis, Pyrazinamide disrupts membrane energy metabolism and targets aspartate decarboxylase, and Ethambutol inhibits arabinosyl transferases involved in cell wall biosynthesis.
