Catheter sclerotherapy for abdominal abscesses, as investigated by the University of Alabama at Birmingham in the ABCS trial, involves the intracavitary administration of chemical sclerosants—specifically ethanol or povidone iodine—following percutaneous drainage. This Phase I/II study evaluates whether these agents can accelerate the resolution of non-fistulous abscesses by inducing an inflammatory response that promotes the adhesion and fibrosis of the cavity walls. Ethanol acts as a potent sclerosant through protein denaturation and cellular dehydration, while povidone iodine serves as both an antiseptic and a chemical irritant. The primary goal of this intervention is to decrease the duration of catheterization and reduce the risk of abscess recurrence in patients with abscesses at least 3cm in size.
