This is a combination therapy used primarily in transarterial chemoembolization (TACE) for the treatment of unresectable liver tumors, such as hepatocellular carcinoma and hepatic metastases. The formulation consists of: - **Ethiodized oil** (also known as Lipiodol or Ethiodol), an iodinated poppy seed oil derivative that acts as a radiopaque contrast agent and drug carrier. - **Doxorubicin**, an anthracycline chemotherapeutic agent that intercalates DNA, inhibits topoisomerase II, and generates free radicals to induce tumor cell death. - **Gelfoam**, an absorbable gelatin sponge derived from purified porcine skin, used as a mechanical embolic agent to block blood flow. In this regimen, doxorubicin is emulsified in ethiodized oil to enhance its delivery and retention within the tumor. Gelfoam is added to further embolize the tumor-feeding vessels. This approach increases intratumoral concentration and half-life of doxorubicin while reducing systemic exposure. The combined effect leads to ischemia (from embolization) plus direct cytotoxicity (from chemotherapy), maximizing local antitumor efficacy[1][6][3].
