Overview

Etomidate + dexmedetomidine is a combination of two intravenous agents used in anesthesia and sedation protocols. Etomidate is an imidazole-derived general anesthetic known for its rapid onset, short duration of action, and minimal cardiovascular depression. It acts primarily as a positive allosteric modulator and agonist at the gamma-aminobutyric acid type A (GABA-A) receptor, enhancing inhibitory neurotransmission in the central nervous system to induce hypnosis and anesthesia. Dexmedetomidine is a highly selective alpha-2 adrenergic receptor agonist that provides sedative, anxiolytic, sympatholytic, and mild analgesic effects with minimal respiratory depression. When used together, dexmedetomidine can reduce the incidence of etomidate-induced myoclonus during induction of anesthesia by modulating neuronal excitability through alpha-2 adrenoceptor activation[6][2]. This combination may be employed for procedural sedation or as part of balanced anesthesia regimens where hemodynamic stability and rapid recovery are desired.
