Overview

EDP is a combination chemotherapy regimen comprising etoposide, doxorubicin, and cisplatin. It is primarily recognized as the standard-of-care chemotherapy backbone for advanced or metastatic adrenocortical carcinoma (ACC), typically administered alongside mitotane (the EDP-M regimen). The regimen's efficacy stems from the complementary mechanisms of its components: etoposide and doxorubicin inhibit topoisomerase II, preventing DNA ligation and inducing double-strand breaks, while cisplatin creates covalent DNA cross-links that impede replication and transcription. Doxorubicin further contributes via DNA intercalation. This multi-targeted approach effectively induces apoptosis in high-grade malignancies but is associated with significant toxicities, including myelosuppression and gastrointestinal distress.
