Cycle B is a specific chemotherapy combination regimen developed by Baylor College of Medicine for the treatment of brain tumors in infants (typically under 3 years of age). It is part of a multi-phase induction protocol that precedes conformal radiation. The regimen consists of oral etoposide administered daily for 21 days and intrathecal mafosfamide administered once weekly. Etoposide is a small molecule topoisomerase II inhibitor that induces DNA double-strand breaks, leading to cell cycle arrest and apoptosis. Mafosfamide is an oxazaphosphorine alkylating agent and a pre-activated derivative of cyclophosphamide; unlike its parent compound, it does not require hepatic activation, which allows for direct intrathecal administration to target leptomeningeal disease and central nervous system malignancies while minimizing systemic toxicity.
