Overview

DA-EPOCH is a dose-adjusted chemotherapy regimen comprising etoposide, prednisone, vincristine, cyclophosphamide, and doxorubicin. Developed by the National Cancer Institute (NCI), it is designed for the treatment of aggressive lymphomas, including diffuse large B-cell lymphoma (DLBCL), Burkitt lymphoma, and T-cell lymphomas. The regimen is characterized by a pharmacodynamic dose-adjustment strategy where the doses of etoposide, doxorubicin, and cyclophosphamide are titrated in subsequent cycles based on the absolute neutrophil count (ANC) nadir of the previous cycle. This approach aims to optimize dose intensity for individual patients while minimizing severe hematologic toxicity. It is often administered as a 96-hour continuous intravenous infusion.
