Everolimus + dalotuzumab is an investigational combination of **everolimus**, a small molecule mammalian target of rapamycin (mTOR) inhibitor, and **dalotuzumab**, a monoclonal antibody targeting the insulin-like growth factor 1 receptor (IGF1R). This combination is designed to inhibit both the mTOR pathway and IGF1R signaling, potentially overcoming resistance mechanisms in cancers driven by PI3K/AKT/mTOR and IGF1R pathways. The pairing has primarily been studied in advanced hormone receptor–positive breast cancer, where combination therapy demonstrated enhanced antitumor activity in preclinical and early clinical studies, particularly in ER-positive, high-proliferation tumors. However, when compared to other combinations (such as ridaforolimus + dalotuzumab + exemestane), clinical benefits for everolimus + dalotuzumab specifically lack large published phase II/III data. The underlying rationale is that mTOR inhibition alone can activate compensatory IGF1R signaling, and dual blockade may provide improved efficacy in selected populations[1][3][4].
