This is a combination regimen of three small molecule drugs—everolimus (an mTOR inhibitor), exemestane (a steroidal aromatase inhibitor), and ribociclib (a cyclin-dependent kinase 4/6 [CDK4/6] inhibitor)—used in the treatment of hormone receptor–positive, HER2-negative advanced or metastatic breast cancer. The rationale for this triplet therapy is to target multiple pathways involved in tumor growth and resistance mechanisms. Ribociclib inhibits CDK4/6 to block cell cycle progression; everolimus inhibits the mTOR pathway to reduce cellular proliferation and survival; and exemestane suppresses estrogen production by inhibiting aromatase, thereby reducing hormone-driven tumor growth. This combination has been studied particularly in patients who have progressed on prior CDK4/6 inhibitors[1][2][3][4].
