Everolimus + palbociclib is an investigational combination of two oral small molecule targeted therapies used in clinical trials for the treatment of hormone receptor-positive, HER2-negative metastatic breast cancer. Everolimus is an mTOR inhibitor that blocks the mammalian target of rapamycin pathway, thereby inhibiting cell growth and proliferation. Palbociclib is a selective cyclin-dependent kinase 4 and 6 (CDK4/6) inhibitor that halts cell cycle progression from G1 to S phase by preventing phosphorylation of retinoblastoma protein. The combination aims to overcome resistance mechanisms seen with endocrine therapy alone or with single-agent targeted therapy, but has been associated with significant hematologic toxicity such as neutropenia and mucositis[1][2][3].
