This combination consists of evocalcet (ASP1517), a small-molecule calcimimetic, and lanthanum carbonate hydrate, a phosphate binder. Evocalcet functions as a positive allosteric modulator of the calcium-sensing receptor (CaSR) on parathyroid cells, thereby reducing the secretion of parathyroid hormone (PTH) without the significant gastrointestinal side effects associated with earlier calcimimetics like cinacalcet. Lanthanum carbonate hydrate works by binding to dietary phosphate in the gastrointestinal tract, forming insoluble lanthanum phosphate complexes that are excreted, thus lowering serum phosphorus levels. The combination is primarily investigated for the management of secondary hyperparathyroidism in patients with chronic kidney disease (CKD) on maintenance hemodialysis, where both PTH and phosphate control are critical.
